Your initial appointment for Botox injection treatments, wisdom teeth removal surgery, or corrective jaw surgery will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be done on the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
IMPORTANT: A patient aged seventeen or younger is considered a minor. An adult or guardian must accompany the patient for treatment. The adult accompanying the patient and the parent(s) are financially responsible for the account. In the event the parents are divorced, the parent accompanying the minor is financially responsible at the time of service, regardless of the divorce decree. The settlement must be resolved between the parents. Ultimately, both parents are responsible for the minor’s healthcare unless the divorce decree specifically states otherwise. For unaccompanied minors, non-emergency treatment will be denied.
If your dentist or physician has taken x-rays prior to your surgery, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility but will require a fee.
